Parent/Guardian and Player survey
Please complete this survey if you are a player of the Werewolves of London Ice hockey club or are the parent or guardian of someone who plays for them.
There are two separate surveys – one for players and one for parents/guardians. These surveys are important so that we know how we are doing and also so that we can feedback this information to our funding sponsors as part of our commitment to them. They are anonymous so please be as honest as you can. Please take the time to do this.
